alvinkonda Hi, Unfortunately, that is not possible directly in Shopify, as the file needs to be stored in a server, and Shopify doesn't have the option to do so. A solution would be to embed a form from a service like JotForms https://www.jotform.com Or simply use a Shopify app like the following https://apps.shopify.com/form-builder-contact-form
